Program Overview:
The 2006 Institute of Medicine report, Preventing Medication Errors, reiterated the belief that errors need to be reported and analyzed if improvements in medication use are to be achieved. Measuring medication safety has long been a challenge since organizational efforts have primarily focused on quantifying medication errors voluntarily reported be practitioners, which at best, uncover just a small fraction of the errors. By its very nature, error reporting is retrospective and reactive to issues. Often, organizations are left with a false sense of security about the safety of their medication systems when using this method alone.
Forward thinking groups recognize voluntary error reporting is just the tip of the iceberg when it comes to understanding medication safety within an organization. Identifying and measuring the level of risk within a system and noting the reliability of processes is fundamental to safety improvement.
Participants will learn how to identify medication safety risk, beyond the use of error reporting, as well as examine various approaches for collecting data including the benefits and limitations of each. A variety of techniques, including proactive, concurrent, and reactive methods will be discussed as well as their advantaged and disadvantages.
