Program Overview:
Healthcare operations are characterized by workers dealing with significant risks every day in a very complex environment where even the smallest mistake could be catastrophic to patients. While the importance of designing safe and reliable systems in healthcare should not be understated, viewing a mishap and its aftermath from the sharp end, at the human contribution to error, is critical- and really interesting!
During this webinar, we will explore why mistakes are the penalty we must pay for our remarkable cognitive skills. First, a basic model of the human brain's information processing system will be presented to help explain both strengths and weaknesses in our ability to perceive information, consider and act on that information, and store and recall memories. The perceptual and cognitive tendencies that influence human behavior will be described, including confirmation bias, change bias, inattentional blindness, hindsight bias, normalcy bias, and others. The focus of the program will be on the conditions, both human and system, that degrade human performance, and strategies to reduce their impact on medication safety. Healthcare managers need to understand the human dimension on their operations if they hope to improve patient safety.

Objectives:

- Identify the differences between the sutomatic mode of cognition and the problem-solving mode of cognition
- List conditions that degrade human performance
- Review perceptual and cognitive biases and how to reduce their negative impact on patient safety
- Describe system design strategies that can prevent or detect human errors or mitigate patient harm from errors
