Fosphenytoin 100mg dose prep

How do you prepare IV fosphenytoin for small q8hr doses (50mg, 100mg)? Lexi-Comp says final concentration should be between 1.5-25mg/mL and given over 50-150mg/minute. Technically, it can be given IV push, but would need a dilution from the 50mg/mL product. Being a hazardous drug, the nurses shouldn't be diluting this on the floor. Do you mix in pharmacy as IV push dose in syringe? Or put in piggyback? For a 50mg dose in a 50mL bag it wouldn't have the concentration recommended by Lexi. Thanks!
