Heparin initial bolus vial, rounding and syringes

Hi everyone,
Looking for an updated assessment on how you are handling accurate dosing of heparin initial boluses. A pharmacist recently noted that nurses are not necessarily using syringes that allow them to accurately measure to our current standard of nearest 100 units (0.02mL) when using the default 5000 unit/mL vial.
Are you:
1) using bolus from bag for initial bolus
2) using 5000 unit/mL vials and if so, what rounding and do you have specific syringes required for admin?
3) using 10000 unit/10mL vials and if so, what rounding?
4) taking another approach? (please describe)

Also, if you have rounding rules in place, is this done automatically by the EHR or are you requiring pharmacists to make adjustments on verification?

Thank you!
Erin
