Zolbetuximab pump programming

Zolbetuximab is a newer monoclonal that utilizes a unique dose-rate in mg/m2/hour. Doses are administered between 100-265 mg/m2/hr based on tolerability and frequency.

The challenge is that Alaris pumps, and likely other pumps, do not offer the ability to directly program in mg/m2/hr. We have built this medication in our guardrails as a continuous infusion with units in mg/hr. This is so nurses only have to complete one dose conversion from mg/m2/hr to mg/hr, verses three conversations of mg/m2/hr to mg/hr to mL/hr.

This is a safety concern because it requires manual calculations, and there is the possibility of inadvertently programming without converting from mg/m2/hr.

Curious if any other institutions are administering this medication and have come up with any creative solutions to improve safety of administration.
