Our health authority has had a few patients admitted on insulin icodec. Our current formulary basal insulin is either NPH or glargine.

How are other institutions managing patient's admitted on weekly icodec?

Are you using patient's own insulin and continuing their home regimen? Are you transitioning the patient to a daily insulin? Or does your institution supply insulin icodec?

Currently we have been managing on a case-by-case basis using patient's own medication and ensuring that regular blood glucose monitoring is ordered but I am curious how other sites are managing this.
