Partial infusions in the Infusion Center

Greetings,

We are working through a challenge related to partial infusions in our Infusion Centers. If a patient doesn't tolerate an infusion and the decision is made to STOP it, how do you document this?
Do you update the dose field on the MAR (concentration mg/mL x volume infused) with a comment indicating the reaction?
Does this effect billing?
