Visual Aids in Medication Lists

In an effort to help our low literacy ambulatory patients understand their medication regimens, we are looking for recommendations on products/programs/resources/strategies that convert their medication lists into a visual chart. Most of the time, these patients have limited access to smart phones so apps we may use for other patients are not sufficient. Please share anything that you have found success with.
