Medication Reconcilliation Ambulatory Services

In planning to implement medication reconciliation at Covenant Health facilities in the ambulatory care setting, we are trying gauge what best practice would be for inclusion/exclusion criteria for the full process of medrec in ambulatory clinics and for perioperative clinics. What decision algorithms were used and was supporting literature for the decisions incorporated?
