PCA and other opioid infusion

I have several questions on safety and security as it relates to injectable narcotics. I would appreciate feedback on any of the following inquiries.

1) Do you have a Pediatric PCA guideline you are willing to share. Interested in pump guardrails for hard and soft maxes, and how you arrived at those numbers
2) Do you require clinicians to order a 4 hour max when placing PCA orders?
3) Does your facility have a process to change the PCA pump access code on a regular basis? If so, can you describe this?
4) Do you secure all narcotic infusion, due to the potential for diversion – does your process differ if the medication is dispensed in a Syringe, bag, or PCA?
