Can you comment on what your organization does to clarify when two orders (with two different medications) exist for the same indication?
For example, we often see an order for acetaminophen for fevers over 38C and ibuprofen when equal to or over 39C. Problem with this is that by the strictest interpretation when a fever of 39C occurs then BOTH acetaminophen and ibuprofen are to be given (if within the proper frequency allowance).
Do you have a phrase you include that says give one first and then followed by the other if condition still exists one hour later? Can you please share?
Similar situation exists in our intubated ICU patients with PRN medications for agitation.
