dual sign off medications

Hello,

I had two questions for everyone:

1) Do the following medications require dual sign-off for nursing at your institution?: Bumetanide infusion, furosemide infusion, sodium bicarb infusion, IV potassium chloride, IV potassium phosphate, pediatric medications?

2) If you're part of a multi-hospital system, do all hospitals have the same dual sign-off requirements or do they vary depending on the hospital and the specific patient population?
