Does anyone have guidelines on concentration of potassium PHOSPHATE IV infusions to be given peripherally?
At my institution, for potassium CHLORIDE, we use a concentration of 10 mEq/100 mL (0.1 mEq/mL) for peripheral administration and up to 20 mEq/100 mL (0.2 mEq/mL) for central lines.
Our EHR (Epic) is set up to default all potassium PHOSPHATE doses to be diluted in 250 mL NS to infuse over 6 hours.
I am wondering about potassium PHOSPHATE doses larger than 15 mMol to be infused via a peripheral line.
For example: KPhos 30 mMol = 44 mEq potassium
44 mEq/250 mL = 0.176 mEq/mL potassium
The question is - should these larger doses be diluted in more fluid (ex. 500 mL)?
I'm unaware of any reports of pain or phlebitis from KPhos doses at our institution.
Would appreciate any feedback and/or guidelines anyone may be using.
