DOAC Discharge Prescription Safety

Good afternoon.
Has anyone had success with EMR build/decision support that improves safety of DOACs (e.g apaxaban, rivaroxaban) as the inpatient orders are converted to discharge prescriptions? A specific scenario I am concerned about is when inpatient orders are entered for apixaban 10mg BID x 7 days followed by 5mg BID, then on discharge the provider inadvertently orders 10mg BID to be continued indefinitely as a discharge prescription rather than the 5mg BID maintenance dose. Does anyone have BPAs that alert a pharmacist or provider? In-basket messages for pharmacist monitoring? Other thoughts?
