High Dose Insulin Euglycemia (HDIE)

Hello,

Our organization has a desire to create an order panel for the treatment of high dose insulin euglycemia (HDIE). The initial thought was to build an infusion record using a 500 unit/500 mL concentration so as to differentiate from our 100 unit/100mL standard concentration bags, while maintaining the same concentration. However, there are concerns with volume overload. We have learned of other sites using a higher concentration such as 10 units/mL (not sure of total volume) or 16 units/mL (250 mL). If your site does use a higher concentration insulin to treatment of HDIE, would you please share what safeguards you have put in place to mitigate errors since this dose would be used very rarely. Thanks!
