Fentanyl infusion in ICU

We are re-evaluating options for continuous fentanyl infusions. Would you mind sharing your preferred fentanyl concentration in ICU ventilated patients? How do you deliver the fentanyl infusion, e.g. PcA pump, LVP pump, other? Thank you for sharing!
