Paxlovid for Inpatient Use

Dear Friends and Colleagues,

ISMP has published a NAN Alert regarding Medication safety issues with newly authorized Paxlovid and the different dosing for normal renal function vs. impaired renal function (one less tablet).

I wanted to start a discussion about if you are planning to use medication as an inpatient medication, how you are going to build medication in your EMR and how are you planning to dispense medication. Are you taking it out of blister pack and repackage it or dispense the blister pack and ask pharmacy or RN to remove the extra tablet for renally impaired patients?
