Pharmacy Driven Vaccination Protocol (Influenza and Pneumococcal)

Does your health system or hospital have a pharmacy-driven vaccination protocol for inpatients (influenza and pneumococcal)? Have you experienced any barriers? Would you be willing to share your protocol?

Our health system EHR generates vaccine orders (influenza & pneumococcal) based off of an admission assessment and vaccination history documented by nursing. If the vaccination history is inaccurate or unknown, a non-optimal vaccine product is ordered necessitating pharmacy intervention. We would like to develop a protocol allowing the pharmacist to change the system-generated order to the optimal product when this occurs.
