Examples of Medication Incident Reports

Hi!

This year I plan to overhaul our hospital medication incident report. I have been asked to include more on actions taken on the incidents. Currently we are not reporting the NCC MERP index but rather just classifying as (no harm, mild harm, moderate harm, severe harm, death). I want to start reporting the NCC Merp index. I also list the event type (incorrect dose, incorrect preparation, omitted dose, etc). I searched the boards and mainly found what people included in their reports, but I am a very visual person. Would anyone be willing to share an example of what the actual report looks like? Something more like a template, I don't need the specific incidents.
