Methadone Processes for Hospitalized Patients receiving opiate agonist therapy

Hi,

We are currently reviewing how methadone is delivered in hospitalized patients for opiate agonist therapy. We are looking to see how other sites across Canada (and the US) deal with methadone including prescribing, dispensing, and administration processes. For example in Saskatoon, its ordered by OAT physician if new start, pharmacy provides narcotic stock in 10mg/mL x 1 mL syringes. Nursing will either access in Pyxis or wardstock if no pyxis and create the dose from the 1mL syringes and document on narcotic record as required. So if 70mg dose, 7 syringes are used. Its treated as High alert, requires an independent double check, then is administered and documented on a paper MAR. We are paper based, some EHR, partial Pyxis.

Secondary question- If your site has CPOE, EHR, eMAR, barcoded medication administration, and ADC's please add this.
