Hi all!
We recently had a patient safety event due to a confusing insulin infusion titration scheme. We start our patient's infusions in units/kg/hr and increase/decrease by units/hr based on subsequent blood glucose results. This causes confusion between our EHR and our pumps. We would like to dose our insulin infusions in units/hr but start the initial rate via a dose banding concept- for example, patients weighing 65 kg to 75 kg would have a starting rate of 7 units/hr. This allows us to avoid units/kg/hr dose in both our EHR and pumps. Does anyone already do this? Do you mind sharing your dose banding table, please and THANK YOU.
