We're updating our inhaled epoprostenol policy and taking a look at newer literature, thinking about efficiencies without losing safety.
One question has come up: has anyone implemented fixed dose inhaled epo administered without a smart infusion pump? Either via Aerogen or other neb system. The thought here is that if there is some acceptable latitude in the dose administered - which it has to be if we are using a fixed dose regardless of weight or other characteristics - then the precision of a smart pump-programmed dose is not necessary. Operationally, it becomes akin to a continuously infused albuterol, though that simplification might provoke some anxiety.
It seems like this would avoid pump availability issues and reduce some risks of inadvertent IV administration/the dispensing steps involved in reducing that risk. Other thoughts and considerations would be great as we work through the options.
