Hello MSOS community.
PROBLEM:
- We are challenged with our outpatient pharmacy system which can not print reduced size symbols for half like 1/2.
- We do not use 1/2 as that could be mis-read as "one or two"
- We have seen a risk with members mis-interpreting "Take one-half tablet...." as 1.5.
- We have reached out to vendors (First Data Bank's Meducation, and ISMP) as well to see what can be done in our system.
PROPOSED SOLUTION:
- We would like to have it read "half" only, but it requires a large amount of resources to program the change and need to confirm community standards before committing to the change.
QUESTION:
- We would like to know the community standard for how fractions are translated (automatically, not by manual pharmacist manipulation) in your outpatient system and what the final pt label would look like. How does the sig read to the members? If you have screenshots or can tell us what operating system you use that would be helpful as well!
- Also, any idea on what you'd do for other fractions like 2/3 or 3/4?
