Adult Hydroxocobalamin (Cyanokit) Prep / Administration for Vasoplegia

Does your institution utilize extended duration infusion of cyanokit (hydroxocobalamin) for vasoplegic syndrome?

How is the medication infused (via smart infusion pump, gravity, other)?

If manufacturer provided tubing is not used, is a filter attached at any point in medication preparation or administration? If so, what size filter is used?
