Transitions of Care Med Rec Infrastructure (BEYOND the Admission Med Rec)

For those of you on Epic, what EMR infrastructure do you have in place and utilize to facilitate medication reconciliation during transitions of care? Some specific questions:

1. What navigators are you using?

2. What is pharmacy’s involvement in discharge med rec?

3. How are you notified in real-time of a discharge?

4. What type of security does pharmacy have to change medication orders at discharge?

5. How do you ensure the discharge med rec is done before the patient discharges?

6. Through what other transitions is a medication reconciliation completed?

- ED to the floor?

- The floor to ICU?

- ICU back to the floor?

- Periprocedural?
