I believe ISMP Canada came out with a recommendation about 20 years ago related to heparin flush products.
We are trying to standardize the orders used to order flushes for trialysis catheters and it occurred to me that it may be an opportunity to ensure our current practice is up to date in regard to catheters--we still lock using high dose heparin. We still use high dose heparin to lock dialysis and apheresis catheters (trialysis and tunneled hemodialysis catheters). Are there recommendations to use lower doses to reduce risk of errors/bleeding? I found a couple articles but was curious what others were doing/seeing and if there was anything I might have possibly missed.
