I would appreciate your guidance regarding a medication safety and system limitation issue encountered in our NICU. We received an order for:
• Insulin Regular 100 units/mL injection.
• Prescribed dose: 0.025 units IV (one-time dose)
• Context: Medication supplied from floor stock, with nursing responsible for preparation and administration.
When attempting to verify the order in Cerner, the system generated the following error: Calculated dose: 0.025 units / 0.00025 mL “A strength less than 0.0001 or a volume less than 0.01 was entered or calculated. Enter a larger value or modify the units.”
As the calculated administration volume (0.00025 mL) is significantly below the system’s minimum threshold, the order cannot be processed through the pharmacy verification workflow.
Have you implemented specific Cerner build solutions (e.g., predefined diluted concentrations, order sets, or minimum dose rules) to safely process these orders? Kindly advise
