Epic EHR--Insulin sliding scale/correctional scale build

Would anyone be willing to share the build out in Epic of their sliding scale/correctional scale insulin careset? We currently are having nurses give the wrong strength of insulin because we currently have the correctional scale all built out under one insulin order with a dosage range 0-28 units and the doses to give in the comment section (low, medium, high scales). Nurses can switch scales if they have 2 high or 2 low glucose readings back to back however we are finding that this doesn't always happen or nurses revert back to the old scale by accident after an adjustment was made previously. My suggestion to the team is to build out the scales separately as 3 different insulin orders with "or" statements.

Other ideas on how to improve this build and decrease the potential for error?
