Integrating Staff Accountability_Just Culture & Remediation

We're working find the right balance with safety & accountability and re-training or remediation & I'd like some help for anyone who wishes to share their process. Happy to link directly with you as well.
We're specifically struggling with the fine lines of retraining, personal accountability and other recommendations to help techs and pharmacist who incur higher than normal error rates compared to their peers. Currently the options may seem either easier or more clear where techs are concerned because there is retraining, re-education and remediation. All of this is in the context of going beyond just culture and implementing process changes which seem to be adequate for the majority of the staff. Please share your process for working through a fair and progressive process with staff to maintain a safe environment based on this.
