Weight Entry Errors

Hello,

We use EPIC as our EMR. We have experienced a few errors recently where an erroneous weight has been entered into our EMR, and subsequently been used to calculate weight-based doses incorrectly - leading to an error. In the first case, it was a decimal point issue where 7 kg was entered instead of 0.7. The second was where the triage RN entered the patient's number of pounds, except entered it as kilograms. We currently have an alert that shows for the front-line clinician who enters an erroneous weight at the time of entry - however, the pharmacist does not see this in verification. In these cases, the alert has been overridden by the frontline clinician. Has anyone found a solution that helps catch these types of errors and alerts the pharmacist while entering or verifying orders?
