I'm looking to see if anyone has had errors in which Exparel is being given pre-op and then receiving a subsequent dose during the procedure or at closure. Specifically targeting patients who may have received the max 20mL (266mg) already, and then received a duplicate 20 mL plus standard bupivacaine.

If you have, what have you done to prevent subsequent errors? Do you have CDS alerts, etc in place or physical barriers? We are utilizing Epic and looking for input on better optimization. Thanks!

We have recently expanded our indications for exparel (liposomal bupivacaine) and have had a few near misses with other products containing local anesthetics within the 96 hour window. In one case a compounded preparation containing ropivacaine was ordered and prepared for a patient who had received exparel pre-op. In another case, an epidural order was placed for a patient with exparel on their profile.

If you have a few minutes, would you please tell us if you are having any trouble scanning Exparel? We are curious as someone reported to us that their system is not able to scan Exparel's barcode as it contains a large amount of characters. We contacted the company but thus far no feedback. We are doing data gathering all the same.
