We were interested in changing the way nursing currently enters heparin into the pumps from units/hr to units/kg/hr but are struggling with the initial entry with a units/hr cap. For example, 18 units/kg/hr with a max dose of 2000 units/hr. If the max dose is exceeded based on weight, then the ordered amount is no longer 18 units/kg/hr, but reduced (i.e. 125 kg patient would then be 16 units/kg/hr to equal the 2000 unit max).

How are other facilities handling their heparin drips (units/kg/hr vs units/hr in pump/nomogram)?
