We have had a number of reports in last month related to Embecta insulin pen safety needles detaching or retracting before administration and coming apart. Upon investigation it seems like the threads on the insulin aspart pen barrels/tips are stripping. The needles can spin endlessly on the pen vs. lock/click in place. The aspart pen threads do not go all the way around the pen like insulin glargine or NPH; instead, there are flat areas which I think allows the pen needle threads to keep spinning and eventually strip.
