Are you aware of a single-dose, or a one-day regimen for dexrazoxane used for the treatment of anthracycline extravasation? If so, could you please share your regimen/reference?

FDA-approved dosing is for a three-day regimen (1000 mg/m2 on days 1 and 2 infused over 1-2 hours, and 500 mg/m2 on day 3).

Does your institution allow a "grace" time window for early administration of PRN medications (e.g. opioids)? Our current policy is that the prescribed time interval must elapse before giving a new PRN dose, but considering a change to allow a "grace" period in light of increasing attention/audits of controlled substance administration.

A) No "grace" period for early administration of PRN medications is allowed - the prescribed frequency time interval must elapse before a new dose can be given.

B) Grace period is allowed - 15 minutes

If using an automated tray replenishment and checking technology (e.g. TraySafe, IntelliGuard), what level of pharmacist oversight of the tray replenishment process do you require?

A) pharmacist checks the labeling/tagging of medications to be placed in tray
B) pharmacist checks the final replenished tray prior to sealing/locking of tray

Dear all:
I know this is not the first time to bring this question, but I tried to search for it in the forum topics and could not find it.
In our hospital I am working to establish a medication safety team/unit, currently I am writing the business case or proposal for it. I am planning to include the number of big hospitals similar to our hospital (Tertiary care leading teaching hospital with around 1100 beds) that have a structured medication safety program/ team / unit with a full time FTEs in the document.
