Does anyone have any practical tips for increasing error reporting in the hospital setting?
Above and beyond providing a Just Culture environment, making reporting as easy as possible, increasing risk awareness, thanking people for reporting and sharing changes made as a result of reporting and providing support as needed for those who self-report or are involved in an issue and other high-level ideas.
Looking for nuts-and-bolts ways to encourage people to report errors and near misses.

optimizing ADR and med error reporting

Hi all! I am not in a medication safety role at my hospital, we currently do not have a formal position for this, but I am assisting with updating our reporting system to provide more useful data with regards to ADR and medication errors. I have a passion for med safety :) I was wondering if there is a standardized definition for preventable adverse drug events? And if anyone has recently gone through a system update and would be willing to let me bounce some ideas off of them I would greatly appreciate it. Thank you so much!

Outpatient Infusion Centers - Error Reporting

Hello all,

If your organization has outpatient oncology infusion centers, do you have any established metrics around error reporting? We are working with a multidisciplinary team on what that would look like and how we can normalize the data to compare sites of different sizes. Some studies report error rates per 100 clinic visits or 1,000 medication orders.

Does anyone have a similar metric for outpatient infusion error reporting?
