We are reviewing our ordering and dispensing process for Kcentra. Hoping to hear what the current practice is at other institutions.

1.Do you round the dose to the nearest vial size according to the nominal vial range (ex: 500 U range) or actual units of factor IX (ex: 569 units)?

Does anyone have any information/experience with the administration of a second dose of KCentra? We're being asked to come up with literature to use in risk assessment and there's just not much out there. Thank you!

Is anyone allowing use of Kcentra when INR is 1.4-1.99 and patient needs emergent surgery? If so, what dosing are you using?

Does anyone put Kcentra vials in their ADCs for administration via IV push? If so, how do you capture the exact number of units administered for billing?
