Medication Calendars

Hi,

We are working on developing processes/policies related to discharge medication calendars. We have criteria at our institution but are attempting to mitigate transcription error risk. Please reply to the below and if you do not complete medication calendars, that information is helpful as well and we would appreciate the quick “No” reply!

1) Institution Name:
2) Do your pharmacists complete mediation discharge calendars?
3a) If yes, is it only certain pharmacists that will do this? Who?
3b) If yes, is it custom made in an out-of-EMR resource or generated within the EMR?
3c) If within EMR, which EMR and which resource?
3d) If custom made, which tool is used (ie: Word/Powerpoint, Canva, other?)
4) Has your organization instituted any strategies to mitigate risks of transcription errors?
