Insulin Pump Refills

Hello,
We are currently standardizing our insulin pump refill dispensing workflow and would like feedback on your approach.

1. Dispense refills from pharmacy or expect patient to provide all insulin (even if formulary)?
2. If patient provides insulin for pump refills, how is insulin vial secured (e.g., in room vs. ADM)?
3. If dispensed from pharmacy, what is used (e.g., 10 mL patient specific vial, insulin syringe(s), or IV syringe)?
4. If IV syringe, how are they labeled/flagged to minimize risk of inadvertent IV or SC administration?
4. What special precautions do you have in place for dispensing U-500 insulin pump refills?
