Med Error Reporting Survey

Hello fellow Med Safety colleagues.

I am currently reviewing our organization's med error reporting culture and looking into ways to improve it.

I am curious to see how our reporting behavior compares to other institutions so am hoping you would be willing to share the following:

1. What is the bed-size and patient population make up of your organization?
2. On average, how many med error reports are received monthly?
3. Are staff encouraged to report near misses in addition to significant events?
4. What strategies have been employed to encourage/improve med error reporting by staff?
