Rectal tacrolimus

We are looking to see if any institutions are using tacrolimus rectally in pediatric patients. If so, and if you are not using a commercially prepared or compounded enema/suppository formulation, how are you administering it in practice?

We would appreciate any insight on preparation, administration technique, dosing considerations, or operational workflow.
