Acyclovir Extravasation

My organization has noticed a potential trend related to acyclovir extravasations requiring hyaluronidase in our pediatric population (particularly neonates) when given through a PIV in the hand. We are currently reviewing if this PIV site is contributing to extravasations or if it is coincidence.

Has any other organizations have had similar extravasations with acyclovir when given through a PIV in the hand? Do you specify which PIV sites are appropriate for medications that are irritants like acyclovir?

Thanks!
