Welcome to www.MedSafetyOfficer.org, a website (“Site”) for the Medication Safety Officers Society (“MSOS”), a membership organization owned by the Institute for Safe Medication Practices (“ISMP”).

MSOS and ISMP are firmly committed to protecting your online privacy. This Privacy Policy is designed to tell you about our practices regarding collection, use and disclosure of information that you may provide to us via this Site. In general, you can visit www.MedSafetyOfficer.org and use some of the functionality of the Site without revealing any personally identifiable information. However, MSOS members must provide personally identifiable information to become a member, and gain access to some of our content and services.

Personally Identifiable Information

The site collects personally identifiable information at the following locations: MSOS Membership, Change of Personal Information Form for MSOS Members, and Listserv sign-up. The information collected includes name, title, home address, business address, practice demographics, and e-mail. We use member contact information from the forms to send official MSOS communications. Users may choose to decline receiving future mailings; please see the "Opting Out" section below. Aggregated demographic information may also be compiled and shared, but does not identify the individual user.

Non-Personal Information Collection

SURVEY INFORMATION

Personally identifiable information is optional in our online surveys and would be used only to contact somebody to clarify an answer. Demographic and profile data that are collected may be analyzed and shared with other organizations and sponsors on an aggregate basis.

E-MAIL

Throughout this site, users are provided the opportunity to communicate with MSOS via e-mail. We read every message and try to reply promptly to everyone. This information is used to reply directly to your questions or comments. MSOS does not sell or rent email address information of our members.

Public Forums, Blogs, and Listservs
 

This site makes discussion threads and listserv subscriptions available to its members, as well as the ability to uploaded documents. Please remember that any information that is disclosed in these areas becomes public information included in internet search engine results. You should exercise caution when deciding to disclose any personal information. These forums are moderated and inappropriate messages will be removed.  Even though ISMP is a federally-certified patient safety organization and MSOS is part of ISMP, information is NOT protected from discovery, if posted on this site.  Be careful.   Also, if you attach any documents (policies, forms, etc.) from your workplace, please make sure that you have your employer’s permission to do so.

Security

MSOS takes reasonable steps, including password protection, to protect the security of personally identifiable information that is transmitted to our Website and to protect such information from loss, misuse, unauthorized access, disclosure, alteration or destruction. Information transmitted over the Internet is not ever completely secure or error-free. Please be aware that email sent to or from this Website may not be secure.

Access, Correction, and Deletion of Personally Identifiable Information

Members have the ability to change personal information previously provided such as name, address, telephone, and e-mail address directly on the Website. All changes must be made in the user controlled change request area. MSOS encourages it members to keep such information up-to-date.  In addition, on a periodic basis MSOS may require the personal identifiable information be updated as a condition of continued membership in MSOS.
