I want to alert everyone that our nurses discovered some 1-L normal saline bags from Baxter that had no barcode printed on the bag to enable BCMA. Please see the attached pictures.

I reported this labeling error to ISMP.

Hi! I was wondering what other sites do for EHR downtimes when reporting out barcode medication administration (BCMA) compliance trends. Requests have been made to tease out BCMA data from known EHR downtimes. This process is manual. I was wondering if sites are speaking to compliance rates, noting trends or noting there is some cushsion in goals to allow for downtimes, etc. Thank you!

Does anybody use barcode scanning for medication administration in ambulatory settings, such as outpatient infusion centers? If so, what EHR platform do you use and when did you implement this technology?

Hi,

We're struggling with our bar code scanning rate for Baxter's "Mini-bag Plus" containers. The bags have a white bar code, and our scanners won't read them if there is fluid behind the bar code or if there is a white label on the other side of the bag.

The only way that we can get the bar code to scan is if we squeeze the bag so that there is air behind the bar code AND also angle the scanner so that the flat water surface is below and serves as the background.

We are currently in the process of evaluating intrathecal medication administration in the outpatient setting, with anticipated use of barcode med administration (BCMA) and provider eMAR documentation. We would be interested to know how your institution handles intrathecal medication documentation in the outpatient setting. If there is a different process for inpatient administration, please comment as well.

Our organization recently shifted from saline syringes being dispensed/owned by pharmacy to them being stored in a pyxis and owned by med materials. Does your organization currently utilize BCMA when administering saline flushes? Prior to this conversion of ownership, we did, but this practice is now being challenged.
