Outpatient Continuous Chemotherapy Compounding Procedures

Good Morning,

I am new to the home infusion patient population (transferring from inpatient practice). I am interested to know if anyone has a SOP regarding continuous infusion chemotherapy compounding practices. For example, for patients receiving a 46 hour continuous fluorouracil infusion, is there a best practice for incorporating intended overfill into the patients order (versus exact dose) to mitigate the need for a coordinated flush to ensure the patient receives the full dose.

BD Optima Infusion Clamps

Interested in hearing if any sites which utilize BD Optima for home infusions, have encountered issues with their Infusion Clamps (N35-0, N40-0).

We’ve encountered situations where the clamp can be placed backwards, which can then force separation between injector and connector, and results in the pump not running correctly. We’ve engaged BD Carefusion on this topic, but would appreciate hearing from other sites if you’ve experienced this challenge as well.

IV cytarabine at home

I was recently approached by an oncologist with the idea to dispense IV cytarabine for outpatient use via an elastomeric pump (Eclipse). We would dispense the Eclipse balls and have the patient (or caregiver) administer this chemotherapy at home, instead of coming into clinic.

Though there is a lot to think about, I could not find any stability data for cytarabine in this closed system. Are any centers currently practicing this process? Is there any information out there I can use to effectively this process?
