Restricted Medication Policies/Lists

Know this topic tends to circulate on this board, but looking to gain some insight into what organizations have for policies that define what medications are restricted to specific areas of the hospital (e.g., ICU only) and/or require special monitoring such as telemetry. Looking for a few good examples of policies and lists that have been effective in driving appropriate patient care/monitoring decisions, without being a barrier to patient care/flow due to being "overly prescriptive". Thanks for your input!

Patient's own meds NOT taken during hospitalization

Hi Everyone,

We have a process for a patient to take their own supply of med(s) in the hospital. We need help with patient meds Not to be taken while in the hospital. The current process is to have the supply go home with family or a care taker. This is not working. So, I am curious to hear what other sites are doing.

CRRT protocols

Interested in what other institutions are doing with CRRT using Primasate 4/2.5, 2/0, or 0/3.5 and Phoxilium 4/2.5.

Do you use ACD? Do you make a separate infusion of calcium chloride? Or, do you add calcium chloride to the bags? Are calcium gluconate bags used intermittently sufficient to maintain calcium levels?

Our IV room gets overloaded with having to make bags with calcium chloride which end up not being used and lots of waste.

I have attached our order set for initiation of CRRT.

Radioactive Infusions and use of Smart Pumps

Our oncology service line will be implementing some new therapies which require infusion with radioactive medications: Lutathera and Pluvicto. I am still in the discovery phase, and I'm looking for anyone who has had experience implementing these. The request from this service line is to purchase an ambulatory infusion pump that our health system currently does not utilize. I'm going through the details to determine if our current pump options will work, which can help for managing drug libraries, service by our biomedical team, and supply purchase standardization.

Sodium Phos 15mMol/250mL add-ease?

Our hospital is not open 24hr, so we have been compounding sodium phos 15mMol/250mL bags to leave available overnight in case it is needed. The vial will fit onto an add-ease bag though, so I'm thinking of supplying them this way instead of wasting them when the compounded ones aren't used. Thoughts on technically having this concentrated electrolyte (15mMol/5mL) vial outside the pharmacy? It would be attached to the 250mL bag via add-ease connector, so not on it's own. Thank you.

NMB In Anesthesia Devices

Just wanted to see what other places are doing regarding NMB in pyxis anesthesia stations, or similar. Currently ours are stored in the matrix drawers, so not "lidded". We have issues with having adequate space in the mini drawers to place these as well due to the controlled substances. Are you all labeling pockets within the matrix, securing in mini drawers, or something else entirely?

Nurse Dual Sign for Thrombolytics

I know this comes up routinely.... Does anyone have a nurse dual sign requirement for thrombolytics? We had a Stroke Certification survey last fall and TJC was pretty insistent that this be a mandatory requirement. Our process has been to evaluate medications based on ISMP guidance so have not had these meds as a requirement. We have alteplase and tenecteplase on our formulary so both would be required. Interested if others have had this 'recommendation' from TJC and added these to their list or just have them on the list. Thank you.
