Care Coordination Model for Med Mngmt

Greetings!

We've been asked to develop a care coordination model for the med management process involving Pharmacy. WE only need to create the model for now, not implement but our internal leaders (clinical, informatics, operations, ambulatory & med safety) feel a bit overwhelmed by how big this seems. Looking at med management points of handoff at various points of thru put for a patient, even given the multiple points of entry seems to be a lot to commit to processes for most patients.

Non-formulary medications in smart pump library

Hi - we just recently went live with B.Braun pump integration and are running into a few IV medications (primarily antibiotics) that are not currently in our smart pumps. Due to low use, we have not added these to formulary, but are concerned that there could be administration errors if they are not built in the pump. How are other institutions handling the addition of non-formulary medications in their smart pump library?

Insulin pen barcode scanning

Dear all:

I would like to take your opinion in the following:
In our hospital we use Insulin pen in the inpatient setting as a patient specific medication devices, when dispensing the medication to the patient we label it with a patient specific dose including patient identification, dose, route frequency and a barcode. and when there is any change in the dose and/or frequency the pharmacy is expected to re-dispense a new pen to the patient labelled with a correct information that is reflected on the medication barcode.

Salt Form Display in Medical Record

How does your EMR display salt forms in the EPIC display name. For example, with calcium carbonate 1,250mg. In the order name in EPIC when the physician is ordering what does it state.

Calcium Carbonate 1,250mg OR

CALCIUM CARBONATE 1,250 MG (500 MG ELEMENTAL CALCIUM) TABLET

We currently have in the administration instructions:
1250mg =500mg Elemental Calcium

Character limits and drug names

Hello,

I am wondering if anyone can share how their facilities display drug names (especially for combination products) when there are character limitations? Some drugs we are struggling with are Genvoya and Stribild, and other multivitamin drugs. If we display enough of the drug names, the strengths get cut off from the label. The current system we use allows the additional display of brand name, which is how we are differentiating between products for now.
