USP 800 - Disposal and Medical Surveillance

Hello Colleagues,

We're a little behind in getting USP<800> to lift-off. We are struggling in some areas more than others in getting this over the finish line.

That would be an education plan for disposal and what to do about medical surveillance ( a recommendation but we still wanted to do "something on some level).

If anyone is willing to share their USP <800> Policy, that would be great!

Stericycle is our primary company for disposal but they don't handle our Chemo.

Our organization recently switched from Privigen to Gammagard and we have had an uptick in skin reactions. Has anyone else had any similar reports when utilizing Gammagard?

Policy for Scanning Medications in the Inpatient and in the Outpatient settings

I am reaching out to see if anyone is able to share their policy for scanning and administering medications in both the Inpatient setting and the Outpatient setting. We are interested in the purpose, bullet points and any clear expectations.

Strength on IV label and epidural percent strength

Looking for recommendations on whether other institutions are mentioning strength of vial on the label itself?

If 2 different percent strength are being used , e.g as in epidural label we are using bupivacaine 0.25 and 0.5%, is this information shown on the label?

IV iron infusion reactions

We have having a significant spike in infusion reactions related to multiple forms of IV iron (iron dextran, iron sucrose and ferumoxytol). We are reaching out to see if anyone else is having the same issues. If so, have you had any luck with interventions? If you are not seeing any increase in reactions, what products are you using, how is is prepared and what are your infusion parameters? Thank you!

Expansion of Med Safety Pharmacist role

I am curious if there are any part-time medication safety officers/pharmacists out there. We are a 600 bed community hospital and I have worked in this position for quite some time and the challenge for the past few years is that the work of a MSOS is difficult to complete on a part-time status and safety initiatives/projects are difficult to initiate/complete in a timely manner or that you simply do not have the time to partake in every project/committee that you would like to.
