Can anyone share their hospital procedures for dealing with non compliant patient on standard medication administration time.

Is their any criteria which gives flexibility to nurses or prescribers on changing the time of administration as per patient preferences?

How this issue is addressed while using bar coding technology?

MAR display for oral liquids and product labelling

I would value input from others that have optimized their Epic display for oral solutions.

Using calcium carbonate as an example, our MAR currently reads:
"Calcium carbonate SALT 250 mg/ml (ELEMENTAL calcium 100 mg/ml)Susp 500 mg : Dose 500 mg …."

Admin instructions include "NOTE: Product is dosed in mg of ELEMENTAL calcium".

Our product label for this dose parallels Epic and reads:
CALCIUM CARBONATE SALT 250MG/1ML ORAL SUSP (5ML)
ELEMENTAL CALCIUM 100MG/1ML (5ML)

ADC Overrides

I am currently reviewing our override list, policy, and procedure, while keeping in mind behaviors and culture shift, I am in process of developing an override decision tree to aid staff in understanding when it is appropriate to override medications.

Does anyone have a decision tree/algorithm they would share related to medication overrides that they keep near their ADC for staff to review? This will also help leadership when coaching staff regarding appropriateness of overriding a medication.

Barcode scanning of bulk items

How do you label and how do nurses scan bulk items such as powders, creams, ointments, and eye drops? We label the tube, container, dropper bottle and once the tube is squeezed, the label is crimped or smeared and there are difficulties in scanning the product for the remainder of the patient's hospital stay. Looking for creative ideas for dispensing, labeling, and scanning of these products.

1. The included medications or medication class
2. If you are willing to share your policy/policies
3. Which EMR you have and any EMR specific build around this you may have performed
