We are about to switch the COVID vaccine that is available within our health-system from Moderna to Pfizer. Our medication management process has pretty much been very similar between locations (academic medical centers to acute care hospitals to clinics). However, we are looking for others that have either had the Pfizer vaccine or had a combination of both. There may be a time in the not-to-distant future when we are asked to juggle all three.

Can anyone share their hospital procedures for dealing with non compliant patient on standard medication administration time.

Is their any criteria which gives flexibility to nurses or prescribers on changing the time of administration as per patient preferences?

How this issue is addressed while using bar coding technology?

Using calcium carbonate as an example, our MAR currently reads:
"Calcium carbonate SALT 250 mg/ml (ELEMENTAL calcium 100 mg/ml)Susp 500 mg : Dose 500 mg …."

Admin instructions include "NOTE: Product is dosed in mg of ELEMENTAL calcium".

Our product label for this dose parallels Epic and reads:
CALCIUM CARBONATE SALT 250MG/1ML ORAL SUSP (5ML)
ELEMENTAL CALCIUM 100MG/1ML (5ML)

I am currently reviewing our override list, policy, and procedure, while keeping in mind behaviors and culture shift, I am in process of developing an override decision tree to aid staff in understanding when it is appropriate to override medications.

Does anyone have a decision tree/algorithm they would share related to medication overrides that they keep near their ADC for staff to review? This will also help leadership when coaching staff regarding appropriateness of overriding a medication.

Do any of your sites have a policy for management of temperature excursions of refrigerated medications? What are your permissible temperature ranges and for which medications (vaccines, biologics, any others)? Looking to ensure our policy is current and supported with literature.

I am working to extend our pharmacy services on our med/surg floors. For anyone with clinical pharmacists in these areas, what daily activities are being performed? What helpful projects have they been able to accomplish? Would like to hear about your successes and challenges in these areas.

How do you label and how do nurses scan bulk items such as powders, creams, ointments, and eye drops? We label the tube, container, dropper bottle and once the tube is squeezed, the label is crimped or smeared and there are difficulties in scanning the product for the remainder of the patient's hospital stay. Looking for creative ideas for dispensing, labeling, and scanning of these products.

Besides evaluation of reported events, how do improve safety with anticoagulants? Do you use triggers? If so, what do you measure, monitor, and report? What successes have you had in preventing harm with anticoagulants?

Wondering if anyone has guidelines for when RPhs and techs should submit med error reports? If the error leaves the pharmacy? If it reaches the patient? If it has the potential to cause serious patient harm? I'm working with Quality to come up with a reasonable standard.
