Is anyone administering nebulized TXA at their facility and have you had problems with fogging? An ED provider ordered it for hemoptysis in my facility and it created so much fog in the patient's room that the fire alarms went off. Is there preferred equipment or methods Respiratory Therapy may be able to provide that could decrease this? Thanks.

proposed guidelines for AUC based monitoring are out, does anyone have experience transitioning from conventional management to AUC? trough levels are still required. what are disadvantages/benefits to AUC hospital wide implementation?

We are trying to translate our service into monetary value. Currently we are looking at ways to estimating the cost of medication error. How else are you measuring your productivity as a service? examples like time spent, system changes, policy changes, etc.?

if patient weight is 40 mg and paracetamol iv dose according to weight is 600 mg ( 15mg/kg) is it okay to give stat dose of 1000 mg for the above patient

If the dose of paracetamol is less than 1000 mg according to 15mg/kg e.g weight is 40 kg
so dose will be 600 mg according to 15mg/kg are you verifying a stat dose of 1000 mg of paracetamol IV for the above patient Looking for your advice thanks

Given the warning for diatrizoate (Gastroview) for risk of severe outcome when this medication is aspirated, do any facilities have any special precautions for patients who are high risk for aspiration? In particular, when Gastroview is administered via an enteral tube, do you have limitations on the rate and/or volume of medication to be administered?

I had two questions for everyone:

1) Do the following medications require dual sign-off for nursing at your institution?: Bumetanide infusion, furosemide infusion, sodium bicarb infusion, IV potassium chloride, IV potassium phosphate, pediatric medications?

2) If you're part of a multi-hospital system, do all hospitals have the same dual sign-off requirements or do they vary depending on the hospital and the specific patient population?
